Developmentally Disabled Service Coordination

Girl-planting-flowersCCSOMO provides individuals with Down’s Syndrome, autism, cerebral palsy or other developmental challenges, and their families guidance and access to resources that meet their distinct needs. These resources include comprehensive medical, social, educational and other specialized services.

Eligibility

  • Currently, this program serves residents of Cape Girardeau County and Scott County only.
  • Individual must have a developmental disability diagnosis prior to the age of 22.

Eligibility for services is determined by the Regional Office of the Missouri Department of Mental Health. To determine if you qualify, please contact the Sikeston Regional Office by calling (573) 472-5300.

Once a referral is made from the Regional Office, a CCSOMO Service Coordinator meets with the individual and family to form a relationship and gather information in order to develop an Individual Service Plan.

Providing Encouragement & Support

Because each individual is unique, we seek to fully develop all options for each person based on individual characteristics and circumstances. We provide encouragement and support for individuals and families in order to promote self-sufficiency, choice and maximum levels of independence.

Service Coordinators locate resources, make referrals, monitor services that are being provided and help assure that our clients’ Individual Service Plans are being followed correctly.
